4. Implementation Requirements

- Manual Entry: If not pre-printed, practitioners must handwrite/print name and registration number
- Legibility: Name and number must be legible and clearly identifiable
- Signature: MC must still be signed personally by practitioner (existing requirement)

- Accountability: Practitioners personally liable for compliance
